Immunizations
Columbia Heights Public Schools immunization enforcement date is Monday, October 21st.
Are your students up to date on their immunizations?
Per Minnesota Law, students either need to be up to date on their immunizations for school or have a signed and notarized exemption form. The day that Columbia Heights Public Schools will begin enforcing immunization compliance will be Monday, October 21st. If your student is still non-compliant or does not have a signed and notarized exemption form on this date, they will not be able to attend school until they receive the needed immunizations or provide an exemption form, and will be sent home from school starting this day if non-compliant. Students who are homeless/highly mobile and who are new to the country will not be excluded from school due to immunization status. If you are unsure if your student falls in this category, please contact your student's school nurse or the District Nurse.

Minnesota law requires that students who are enrolled in public schools provide documentation of vaccinations for certain communicable diseases (or a legal exemption). Students must be current on their immunizations or have a legal exemption in order to be enrolled in school. Questions regarding immunizations can be directed to your student’s health office or the District Nurse at 763-528-4580.
