Immunizations

  • Minnesota law requires that students who are enrolled in public schools provide documentation of vaccinations for certain communicable diseases (or a legal exemption). Students must be current on their immunizations or have a legal exemption in order to be enrolled in school. Questions regarding immunizations can be directed to your student’s health office or the District Nurse at 763-528-4580.

Park Nicollet Immunizations

  • CHPS partners with Park Nicollet to provide immunization-only appointments for state-required school vaccinations. If you would like to make an appointment, you can either go to https://www.healthpartners.com/care/everyday/kids-health/no-shots-no-school/ and click “schedule now,” or you can go to the clinic for a walk-in appointment.

    What you need for your appointment:

    • To fill out the “barrier-free immunization form” (linked below)
    • A photo ID
    • To tell them that your student goes to Columbia Heights Public Schools
    • To bring any immunization records you have
