Minnesota law requires that students who are enrolled in public schools provide documentation of vaccinations for certain communicable diseases (or a legal exemption). Students must be current on their immunizations or have a legal exemption in order to be enrolled in school. Park Nicollet Immunizations
CHPS partners with Park Nicollet to provide immunization-only appointments for state-required school vaccinations. What you need for your appointment:
- To fill out the “barrier-free immunization form” (linked below)
- A photo ID
- To tell them that your student goes to Columbia Heights Public Schools
- To bring any immunization records you have
